As the initial step in our process, we need to get the required documentation about your dental practice. Click here to upload documents.

The Assessment

After you submit all the required documentation, we'll start our assessment. This process takes 10 business days upon receipt of documents.

Once our assessment is ready, we'll send you a meeting request to review the results and answer any questions.

Business Valuation Information Request List

Following is a list of information and documentation we'll need from you. 

Last Three (3) years’ US Corporate Tax Returns, please include all pages

If last year’s Income Tax Return is not yet available, please provide a 12-month Profit & Loss statement covering that period. In addition, include a copy of the IRS filing extension.

 

Also, submit your most recent trailing twelve months (TTM) Profit & Loss statement.

From Office Management Software: 

Summary Production-Collection Report month by month for the previous last 12 months. 

Summary Production by Procedure Report for the same period as above

Summary Production by Provider Report for the previous last 12 months. This report requires a roster of your clinical staff to help identify who belongs to each provider code.

Provide a Patient Statistics Report covering the most recent 18-month period. The report should be calculated by selecting the latest month-end available and then going back 18 months from that date.

Please provide a copy of the signed Lease Agreement, including all executed extensions and amendments to the original lease. This documentation may be submitted at a later stage if the listing is formally assigned to the Broker.

Please complete the attached Primary Practice Information Form to the best of your knowledge.
